There is a strange ghosting effect between each frame that's making me nauseous, and any action scene is severely pixelated. This is clearly a single pass encoding job.

If you think this is perfect, your standards are very low indeed. I know a bad rip when I see it. Let me explain it to you: a single pass encoding gives the same bitrate to scenes with no motion (meaning they look fine) as to scenes with high motion (meaning they are overpixelated). A 2-pass process scans the video first to determine which scenes need more bitrate and which need less. A 2-pass encoding is MANDATORY, as specified under the Codec section of the releasing standards:
